MedicationsIf you are on medications, bring them with you to your visit. If you are on medications that require level monitoring, like coumadin or warfarin, bring a copy of your most recent lab test results.

Vision – If you require correction to your vision, whether or not you routinely wear contacts, you must bring a pair of glasses with you to the exam. If you have retinopathy, cataracts, aphakia, macular degeneration, or other eye problems, an eye specialist (optometrist or ophthalmologist) should complete the vision portion of the DOT physical form. We have copies of these forms and have a form letter for the doctor to make this an easier process.

Hearing – If you wear hearing aids please bring them and extra batteries.

Diabetes – If you have Diabetes and you take an incretin mimetic (ex. Byetta), are not well controlled (A1c >10), or have a history of hypoglycemic reactions, you are required to bring a letter from your treating doctor, BEFORE your CDL Exam; with specific areas addressed to include a 1-month log of fasting blood sugars and most recent HgA1c. Please call our office for assistance with this Consult. We have a form letter for the Dr. to make this an easier process. Bring in all diabetes medications and fasting blood sugar log over the last month. If you use insulin, you must apply for a waiver and bring documentation from an endocrinologist indicating well controlled glucose.

Cardiac – If you have had a Heart Attack, MI, Stents, angioplasty (ballooning), Angina, heart failure, or valvular disease, you will require a cardiology consult BEFORE your CDL exam. Please call our office for assistance with this Consult. We have a form letter for the Dr. to make this an easier process. You will need to bring a copy of your most recent EKG and your last exercise stress test results. Also, if you have had an echo, chest X-ray, or holter monitor, bring these results. Based on condition and severity, further testing or documentation may be required, so call prior to your appointment for more detailed information about your specific condition. If you have a pacemaker, you will need to bring documentation of annual pacemaker check.

Mental Health – If you are currently being treated for Depression or other mental illness or have adult ADHD or ADD and are being treated for this we need a consult from your treating PCP or mental health provider with specific information, BEFORE YOUR CDL EXAM. Please call our office for assistance with this Consult. We have a form letter for the Dr. to make this an easier process.

Stroke/Seizure/Brain Injury – If you have had a Stroke, Seizure, Traumatic brain injury, or TIA you will require a Neurology consult BEFORE your CDL exam. Please call our office for assistance with this Consult. We have a form letter for the Dr. to make this an easier process.

Lung/Respiratory – If you have a history of any specific lung disease or are a smoker over 35 years of age, spirometry testing is required. Please bring documentation of this testing or let us know that you will need this testing to be done during your physical when you call to make an appointment. We can perform this testing in our office but need to allow sufficient time for your appointment to perform this extra testing.

Sleep Apnea – If you have Sleep Apnea or Sleep Disorder we need a consult from your sleep specialist or PCP with specific information. BEFORE YOUR CDL EXAM. Please call our office for assistance with this Consult. We have a form letter for your Dr. to make this an easier process. If you use a CPAP machine, we will need to see usage statistics for the last month. Your company that manages the CPAP machine can do this for you.
